How to Apply to Camp Towhee

1. Parents should call Integra and ask the Intake Worker for a camp application package. (Referring persons may request a package for parents who need assistance completing the forms).
   
   The application package consists of:
• A Parent Questionnaire
• A Teacher Questionnaire: Parents give this to the teacher to complete and forward to Integra.
2. In order to establish the presence of a specific learning disability, we require a psychological assessment conducted by a Registered Psychologist. The application package includes a Request for Psychological Information form. Once the parents have signed this, they send it to the Psychologist who will then forward the assessment to Integra.
